HEALTH NAVIGATION CONSULTATIONS

We are currently creating a resource map for the CLSC in Parc-Extension and will begin health navigation consultations 

October 2019.

 

During these consultations, a team of multidisciplinary CIME students will meet with an asylum seeker, record a detailed history and answer questions. The team will then direct the asylum seeker to the appropriate services and professionals to better address their requests, while providing the option of accompanying the individual to access services if needed.
